摘要:
目的 探究右侧非血栓性髂静脉压迫综合征(NIVCS)的临床和影像特征以及腔内治疗的疗效。方法 回顾性分析了2019年1月至2024年2月江苏大学附属南京市高淳人民医院下肢静脉造影的影像资料,选取一组临床上诊断为右侧非血栓性髂静脉压迫综合征并且行腔内治疗的患者,分析其临床及影像特征、技术成功率、术后临床改善情况及支架通畅率。结果 中位随访时间为 30.2个月(3～58个月),共24 例患者(年龄42～73岁)纳入本研究,其中男性比例明显高于女性[男性:女性=22(91.7%):2(8.3%),P<0.001]。根据CEAP(clinical etiologic anatomic pathophysiologic)分级,C3级14例,C4级9例,C6级1例。影像显示患者最常受压的部分是髂外静脉近心端占62.5%(15/24),其次为髂总静脉分叉段近心端为20.8%(5/24),外压结构最常为髂内外动脉58.3%(14/24),其次为髂外动脉与骶骨20.8%(5/24)。技术成功率100%(24/24),且每例患者均仅植入1枚支架(直径12～14 mm,长度80～90 mm),其中最常应用的支架类型为Smart control,占79.2%(19/24)。随访期间,患者水肿缓解率为85.7%(18/21),溃疡的愈合率100%(1/1),所有支架均保持通畅。结论 右侧非血栓性髂静脉压迫综合征更常见于男性患者,因受压部位常位于髂静脉中远心端,术中植入支架可能很少影响对侧髂静脉血流,血管腔内治疗是一种安全有效的方法。
