【摘要】 目的 比较明胶海绵颗粒、明胶海绵条与弹簧圈联合明胶海绵颗粒在脊柱肿瘤术前介入栓塞中的疗效和安全性。方法 收集2012年6月至2022年6月在复旦大学附属中山医院及厦门院区接受脊柱肿瘤手术前介入栓塞治疗的92例患者临床资料。根据介入术中使用栓塞材料不同,将患者分为明胶海绵颗粒组(n=36)、明胶海绵条组(n=36)及弹簧圈+明胶海绵颗粒组(n=20)。采用Kruskal- Wallis检验比较3组间术中出血量,Fisher精确概率检验比较3组间并发症发生率。结果 所有患者手术切除术均成功实施。明胶海绵颗粒组、明胶海绵条组、弹簧圈+明胶海绵颗粒组术中中位出血量分别为400(200~950) mL、1 000(513~2 000) mL、1 000(500~1 350) mL,明胶海绵颗粒组术中出血量明显低于明胶海绵条组(P=0.000 8)及弹簧圈+明胶海绵颗粒组(P=0.002 7),差异有统计学意义。术后明胶海绵颗粒组有4例患者出现切口感染/局部脓肿形成,明胶海绵条组有1例出现切口裂开,弹簧圈+明胶海绵颗粒组有1例出现局部血肿形成。3组间并发症发生率差异无统计学意义(均P>0.05)。结论 明胶海绵颗粒、明胶海绵条、弹簧圈应用于脊柱肿瘤介入栓塞均安全有效。在减少外科手术中出血方面,超选择插管使用明胶海绵颗粒栓塞肿瘤供血支的疗效优于非超选择插管使用明胶海绵条或弹簧圈联合明胶海绵颗粒栓塞。
