【摘要】 目的 探讨LACbes左心耳封堵器植入术后固定盘被挤压呈高柱状形态的安全性和有效性。方法 回顾性分析2020年至2021年在上海周浦医院接受LACbes封堵器植入术治疗的非瓣膜性房颤患者临床资料,记录术后封堵器固定盘呈高柱状形态患者的基线特征,围手术期、术后45 d和12个月经食管超声心动图(TEE)检查情况,术后6个月不良事件。结果 150例患者植入LACbes左心耳封堵器。其中术后固定盘呈高柱状形态患者15例(10%),年龄为(70.2±8.9)岁,房颤血栓栓塞风险评分(CHA2DS2-VAS)为(5.3±0.9)分,抗凝治疗出血风险评估(HAS-BLED)评分为(3.4±0.6)分,TEE检查示锚定区直径为(15.1±3.1) mm;6例患者左心耳形态呈仙人掌形态,4例呈菜花形态。15例患者均一次成功植入LACbes,围手术期、术后45 d无并发症发生。术后12个月1例患者并发器械相关血栓(DRT),调整抗凝药物剂量治疗2个月后 DRT消失;另1例术后90 d出现心脏压塞,行心包穿刺引流,结合相关检查考虑为心肌损伤综合征,给予激素治疗后随访9个月未见心包异常。未发生血栓栓塞、出血、死亡等不良事件。结论 LACbes左心耳封堵器植入术后固定盘形态呈高柱状是安全的,且有良好的封堵疗效。LACbes封堵器适用于锚定区直径较小的左心耳,术中无需换用其他类型封堵器。
