【摘要】 目的 探讨神经介入治疗对脑血管病伴糖尿病患者肾功能的影响。方法 回顾性分析2013年3月至2016年3月采用神经介入术治疗的108例脑血管病伴2型糖尿病患者临床资料。所有患者介入术中对比剂用量<250 mL,观察术前、术后24 h血清肌酐(sCr)、血清胱抑素(Cys)C水平变化,根据肾脏疾病膳食改良(MDRD)公式和Larsson公式分别计算肾小球滤过率估计值(eGFR)。结果 与术前相比,所有患者术后24 h sCr、血清Cys C水平均显著增高(P<0.000 1,P=0.015),eGFR明显降低(MDRD公式 P<0.000 1,Larsson公式 P=0.021),均未出现需要治疗的肾损害。结论 神经介入术对比剂应用可引起2型糖尿病患者肾功能下降,sCr、血清Cys C联合测定有助于尽早发现对比剂应用后肾功能改变,对比剂常规剂量对2型糖尿病患者神经介入手术是安全的。
