 【摘要】 目的 探讨原发性肝癌患者血清中高尔基体膜蛋白-73（Golgi protein?蛳 73，GP73）的表达水平及其早期诊断价值。方法 收集人外周血血清490例，应用酶联免疫定量测定法和电化学发光法分别检测血清中GP73表达和AFP水平。结果 肝细胞肝癌、胆管细胞癌及布加综合征合并肝细胞肝癌患者血清GP73的表达水平均高于其他各组（P < 0.05）；GP73、AFP诊断肝细胞肝癌（包括布加综合征合并肝细胞肝癌）的受试者工作特征曲线下面积、灵敏度、特异度、正确率及最佳临界值分别为0.868、90.8%、75.5%、77.6%、43.40 ng/ml；0.739、60%、83.2%、80.2%、20 ng/ml。GP73诊断肝细胞肝癌的受试者工作特征曲线下面积及灵敏度显著高于AFP（P < 0.05）；特异度及正确率差别无统计学意义（P > 0.05）。GP73诊断胆管细胞癌的受试者工作特征曲线下面积为0.774，灵敏度、特异度及正确率分别为88.6%、72.7%及73.9%，最佳临界值为45.40 ng/ml。结论 血清GP73对原发性肝癌的诊断价值优于AFP，有望成为原发性肝癌早期诊断的血清标记物。
