摘要:
目的 比较脉冲场消融(pulsed field ablation,PFA)与冷冻球囊消融(cryoballoon ablation,CBA)治疗心房颤动(atrial fibrillation,AF)的有效性及安全性。方法 检索PubMed、Web of Science、Cochrane及Embase数据库获得相关文献,检索时间从建库至2024年5月31日。使用Stata 18.0软件进行Meta分析。结果 共纳入了12篇文献。AF患者3 765例,采用PFA治疗1 430例,采用CBA治疗2 335例。PFA组与CBA组相比,总手术时间(MD=-0.85,95%CI:-1.43～-0.28,P=0.004)、膈神经损伤(OR=0.09,95%CI:0.04～0.24,P<0.001)、食管损伤(OR=0.20,95%CI:0.04～0.90,P=0.036)方面,差异有统计学意义; X线透视时间(MD=0.31,95%CI:-0.02～0.63,P=0.066)、房性心律失常复发率(OR=0.73,95%CI:0.53～1.01,P=0.057)、心包填塞(OR=2.37,95%CI:0.97～5.81,P=0.058)方面,差异无统计学意义。年龄亚组分析显示,在≤65岁患者中,PFA在减少房性心律失常复发率方面,差异有统计学意义(OR=0.61,95%CI:0.42～0.89,P=0.01); 在>65岁患者中,差异无统计学意义(OR=1.04,95%CI:0.69～1.56,P=0.853)。结论 与CBA相比,PFA手术时间更短、膈神经及食管损伤更少,但X线透视时间、房性心律失常及心包填塞等差异无统计学意义。
